If something goes wrong, tell us.

You have the right to raise a concern about your care, your privacy, your bill, or how you were treated — and to have it taken seriously. Filing a grievance will never affect your treatment, your discharge, or anything we report to a court or referrer.

What happens next

    You tell us

    Use the form below, call the office, or tell any staff member. You do not have to use a particular form of words, and you do not have to give your name.

  2. 2
    We acknowledge — within 5 business days

    You get a reference number immediately. A director or the office manager confirms we have it and tells you who is looking into it.

  3. 3
    We investigate

    We talk to the people involved, read the record, and write down what we find. If your grievance is about a specific staff member, that person does not investigate it.

  4. 4
    We respond — within 30 days

    You get the outcome and any change we are making because of it. If it will take longer than 30 days, we tell you why and when to expect an answer.

You can also go straight to a regulator

You are never required to use our process first. Complaints can go directly to the NJ Division of Mental Health and Addiction Services, or — for privacy or discrimination — to the U.S. Department of Health and Human Services, Office for Civil Rights.
